OTS Intake Assessments

Joint Comprehensive Assessment Plan (J-CAP)

J-CAP is a four-part electronic assessment completed by staff to aid in case management and discharge planning practices. The four parts are: Intake Assessment, Progress Review, Pre-release Interview and Discharge Plan.  J-CAP was developed as an electronic system that allows the NJDOC to enter, update and review inmate information in an electronic data bank. The implementation of J-CAP across the state will standardize many social service practices and forms and will assist in the coordination of information shared between institutional departments and across state agencies. J-CAP was implemented in 2009 statewide.

Level of Service Revised (LSI-R)

In 2005 the NJDOC implemented the LSI-R as the risk needs assessment for the department.  The Level of Service Inventory -Revised (LSI-R) is a risk needs assessment identified by reentry authorities as a best practice approach to measure inmate’s risk of recidivism.   To ensure all inmates are given an assessment NJDOC has developed policies to establish the LSIR assessment as a part of the institutional intake process.
